2. Call the ODHS Hotline: Calling the ODHS support hotline at 800-699-9075 is an additional way to start the replacement benefits application process.
The official Request for Replacement Form (Form DE 0349D) is available for SNAP beneficiaries to download and fill out as well as submit online.
The deadline for Oregon residents to apply for SNAP replacement benefits
You have to request replacement SNAP benefits within ten days of the event that caused your food to be lost or damaged.
